What we collect
- Contact details you send through booking, proposal or event forms.
- Health information you share on the intake form, such as conditions, medications and fall history.
- Payment details, which are handled entirely by Square. This site never sees or stores card numbers.
How health information is protected
- Intake and waiver forms are sent over an encrypted connection to a HIPAA-compliant form provider that has signed a Business Associate Agreement (BAA) with Core Wellness Solutions.
- Health information is never stored in your browser, in email, or in website analytics.
- Only Tavia can access your records. Paper notes are kept locked.
- We do not sell or share your information. We disclose it only with your written permission, or when the law requires it.
Your rights
- Ask for a copy of the information we hold about you.
- Ask us to correct or delete it.
- Withdraw permission for photos at any time.
Event sign-in sheets
At group events, sign-in collects first names only for attendance counts. These lists hold no health information.
Cookies and maps
This site uses an embedded Google Map and Google reviews, which may set Google cookies. Booking pages are provided by Square under Square's privacy policy.
